Foot Care Nurses are trained to look after the foot health of every client, including those that are considered high risk for foot complications such as people with diabetes, kidney disease, alterations in sensation and circulation problems.  
 
A visit to the foot care clinic consists of a comprehensive foot health management program.  The initial visit will include:

•    A thorough Health Assessment
•    Treatment for current foot concerns as well as preventative measures i.e. nail cutting & filing, corn & callous care, involuted & ingrown nails
•    Education
•    Referral
 
For People with Diabetes
 
People with diabetes need to take special care of their feet in order to reduce complications including amputation. While the best way to reduce the complications is to control your blood glucose levels, there are other ways to take care of your feet.  We can show you the steps you can take to keep your feet healthy for life.
 
While we specialize in nursing foot care, we are nurses first and therefore, can address and provide care for other health concerns.
 
 
 
 
 
 
 
 
 
Our care management services include:
 
•    Comprehensive health assessment of the client*s physical, psychological,
social, environmental, and financial status and ability to function
•    Develop and utilize nursing diagnoses and problem statements
•    Development, implementation, monitoring, and modification of a plan of care
•    Linking the client with the most appropriate institutional or community resources,
advocating on behalf of the client for scarce resources, and developing new resources
•    Screen, arrange, and monitor in-home help or other services
•    Coordination of service providers
•    Provide crisis intervention
•    Act as a liaison to families at distance
•    Education of the client, the family, and the community
•    Monitoring the client*s progress toward goal achievement and periodic reassessment of changes in health status
•    Monitoring the service plan to ensure the quality, quantity, timeliness, and effectiveness of services
•    Assist with moving an older person to or from a retirement complex, care home or nursing home evaluation of client and program outcomes
•    Provide consumer education and advocacy
•    Offer counseling and support
